It's pretty common, yes. 
There are also advertisers that don't want traffic from ExoClick, but they are totally fine with trafficjunky for example.

Why? 
I have no clue, maybe they have had bad experiences with those sources?

Maybe that domain is owned by the offer owner and they don't want you popping ads over their site to steal their earned traffic (seo or otherwise).

Who knows? 

That has nothing directly with advertising networks (most likely).

Looks to me like some inside grudge or dispute possibly?
