  1. Pills!
  2. The pick-ups which are NOT items, are the Pills, Tarot Cards and Trinkets.
  3. These will more than usually harm you than help you.
  4. Let's start with the pills!
  5. The pill's aren't actually guaranteed to be a certain stat up. They all differenciate.
  6. These go into your Q slot and are one-time-use items.
  7. Once you find the effect of said pill/effect, then it's easier to avoid the ones that are bad.
  8. Here are the effects of pills.
  9. Bad Gas: Poisons any enemies in a small area around you.
  10. Bad Trip: Causes you to lose one heart, but if you only have half of a red heart, it will turn into a full health pill.
  11. Balls of Steel: Gives 2 spirit hearts.
  12. Bombs are Key: Swaps your bombs and keys.
  13. Full Health: Gives you full health. Duh.
  14. Health Down: Decreases your health by one.
  15. Health Up: Increases your health by one.
  16. I found pills (and ate them!): Isaac will make a 'Durr' noise and will have apparent changes.
  17. Pretty Fly: Gives an orbiting fly which protects you from flies and tears.
  18. Range Down: Decreases range.
  19. Range Up: Increases range.
  20. Speed Down: Decreases speed.
  21. Speed Up: Increases speed.
  22. Tears Down: Decreases tears.
  23. Tears Up: Increases tears.
  24. Telepills: Teleports you to a random room, and has a small chance to teleport you to the I AM ERROR room!
  25. Wrath of the Lamb Pills!
  26. Luck Up: Increases luck.
  27. Luck Down: Decreases luck.
  28. Friends Till the End!: Spawns 3 Kamikazie flies which do significant damage.
  29. Puberty: Isaac has hairs growing from his head.
  30. Congrats! You now know the pills!
  31. Tarot Cards and Minor Arcana!
  32. Tarot cards are cards which can help you or destroy you. BUT, learn what they do, and you can avoid that! These cards are also in your Q slot. If you have a pill when you pick up the card, you will drop said pill and keep the card. You can re-pick up the
  33. Let's start, shall we?
  34. O The Fool: Teleports you to the spawn room.
  35. I The Magician: Gives you homing tears for one room.
  36. II The High Priestess: Spawns down Mom's food for one room and deals damage to all enemies. (Note, if there are no enemies in the room, the foot will go for you)
  37. III The Empress: Turns the player into a Demon and gives you more damage and tears for a room.
  38. IV The Emperor: Teleports the player into the Boss Room.
  39. V The Hierophant: Drops two soul hearts for the player.
  40. VI The Lovers: Drops two red hearts for the player.
  41. VII The Chariot: Gives you temporary invincibility and lets you damage enemies.
  42. VIII Justice: Drops one key, one bomb (troll or normal) one heart (spirit or normal) and a tarot card or pill.
  43. IX The Hermit: Teleports you to the Shop.
  44. X Wheel of Fortune: Spawns a Slot Machine.
  45. XI Strength: Gives the player +1 to all stats for a room.
  46. XII The Hanged Man: Gives the player flight for 1 room.
  47. XIII Death: Damages all enemies in the room.
  48. XIV Temperance: Spawns a Blood Donation machine.
  49. XV The Devil: Gives you the Book of Belial effect for one room.
  50. XVI The Tower: Spawns six random troll bombs in the room.
  51. XVII The Stars: Teleports the player to the Item Room.
  52. XVIII The Moon: Teleports the player to the Secret Room.
  53. XIX The Sun: Damages all enemies in the room and gives the player full health.
  54. XX Judgement: Spawns a beggar. (Can be a health or coin beggar.)
  55. XXI The World: Reveals the entire map and secret room.
  56. Minor Arcana cards!
  57. The Joker: Teleports you to the Devil Room
  58. 2 of Hearts: Doubles your health.
  59. 2 of Spades: Doubles your keys. (If none, it will give you two.)
  60. 2 of Clubs: Doubles your bombs. (If none, it will give you two.)
  61. 2 of Diamonds: Doubles your money. (If none, it will give you two.)
  62. You now know the bad and the good of Tarot Cards!
  63. Trinkets!
  64. Now, let's move onto the more interesting side of the game. By that, I mean one of my favorite things about the game, the trinkets!
  65. Let's start, shall we?
  66. ???'s Soul: This gives you a familiar that shoots homing tears.
  67. Ace of Spades: This trinket increases the chance of getting tarot cards.
  68. Bloody Penny: Gives a chance to drop a half heart when a coin is collected.
  69. Burnt Penny: Gives a chance to drop a bomb when a coin is collected.
  70. Cain's Eye: When starting a floor, there is a chance that you will get the Compass effect.
  71. Cancer: Decreases tears delay by 2.
  72. Child's Heart: Increases the chance of a heart drop at the end of a room.
  73. Counterfeit Penny: Gives a chance to get an extra coin when a coin is collected.
  74. Cursed Skull: If the Player has less than 1 full heart, after taking damage they will be teleported to the previous room.
  75. Curved Horn: Increases damage by 2.
  76. Eve's Bird Food: Chance to spawn the Dead Bird when an enemy dies.
  77. Flat Penny: Chance to get a key when a coin is collected.
  78. Fish Head: If you get hit, you spawn attack flies.
  79. Goat Hoof: Increases the Players speed.
  80. Isaac's Head: Gives the Player a severed Isaac head which fires piercing tears.
  81. Isaac's Fork: Clearing rooms have a chance to heal Isaac.
  82. Judas' Tongue: Items in the Devil Room only cost 1 heart.
  83. The Left Hand: Replaces all chests with Red Chests.
  84. Liberty Cap: Chance to give Isaac a random passive effect from the mushrooms.
  85. Lucky Toe: Increases the chance to get a drop at the end of the room.
  86. Maggy's Faith: Gives an eternal heart at the start of every floor.
  87. Match Stick: Increases the chance of bombs dropping at the end of a room.
  88. A Missing Page: A chance to induce The Necronomicon's effect upon taking damage.
  89. Mom's Pearl: Grants a chance to drop a non-specific Heart drop into a Soul Heart.
  90. Pinky Eye: Chance to poison an enemy.
  91. The Polaroid: If you have half a red heart, you will recieve a shield for five seconds. (Note: Best used with Soul Hearts)
  92. Push Pin: Chance to fire a spectral and a piercing tear.
  93. Red Patch: Chance to increase damage upon getting hit.
  94. Rusted Key: Improves the chance of keys and golden chests dropping at the end of a room.
  95. Safety Cap: Improves the chance of getting pills at the end of the room.
  96. Samson's Lock: Chance to increase damage each time an enemy dies.
  97. The Tick: Drain's bosses health by 15% but, you can't change your trinket once you pick it up.
  98. Umbilical Cord: When Isaac is at low health, a Little Steve familiar spawns.
  99. Congrats! You're almost an expert on all these items!
  100. Passive Items!
  101. Passive items are some of the best things about this game. Although, some of these items... Are REALLY damn bad.
  102. Let's get started, shall we?
  103. 1-UP!: This item will give you an extra life, and you will spawn with the same amount of health you died with.
  104. 3 Dollar Bill: This item will give you a random tear effect every room.
  105. 9 Volt: Gives you 2 ticks of your active item all the time.
  106. A Dollar: Gives you 99 cents.
  107. A Lump of Coal: The amount of time your tears stay on the screen increase your tears damage.
  108. A Quater: Gives you 25 cents.
  109. Abel: Mirror's you, and shoots low damaging tears.
  110. Ankh: When you die, you respawn as ???
  111. Blood Bag: Gives you a health up, a small speed up and refills 5 hearts.
  112. Blood of the Martyr: Damage up.
  113. Bloody Lust: Every enemy you kill increases your damage.
  114. Bob's Curse: Gives you five poison bombs.
  115. Bobby Bomb: Gives you five homing bombs.
  116. Bomb Bag: Occasionally drops bombs at the end of rooms.
  117. Boom!: Gives you ten bombs.
  118. Box: Drops one of every drop (one key, one bomb, one coin, one heart, one tarot card, one pill and one trinket)
  119. Breakfast: Gives you one health up.
  120. Brimstone: Replaces your tears with a large damaging beam, requires charging.
  121. Bucket of Lard: Gives you two heart containers, lowers speed and fills half a heart.
  122. Cat-O-Nine-Tails: Shot speed up, NOT a tears up!
  123. Celtic Cross: Gives you a chance when you get hit to spawn a sheild.
  124. Charm of the Vampire: Every 13 kills will fill a half heart.
  125. Chemical Peel: Gives you a damage up on one side of your face. (If used with Cain, it will randomly fire the damaging shot.)
  126. Chocolate Milk: Allows you to charge your tears for high damage, or rapid fire your tears.
  127. Cube of Meat: Level one gives you an orbital, level 2 gives you a firing orbital, level 3 gives you a Meatboy and level four gives you a high damaging Meatboy!
  128. Cupid's Arrow: Gives you piercing shots which go through enemies.
  129. Daddy Longlegs: Spawns a spider leg to come down and crush enemies. Very high damage.
  130. Dead Bird: If you get hit, you will spawn a Bird which will damage enemies very rapidly.
  131. Dead Cat: Gives you 9 lives and sets you to 1 heart.
  132. Dead Dove: Gives Isaac spectral tears and flight.
  133. Dessert: Gives you one health up.
  134. Dinner: Gives you one health up.
