First of all :

This is a great addon which helps you delete cookies for sites separately with ease. Very useful for many other things as well and extremely simple to use.
Secondly :
FoxyProxy - http://getfoxyproxy.org/
This is for changing proxies, it allows you to manage proxies and switch between them easily.
Thirdly :
Better Privacy - http://nc.ddns.us/extensions.html
This deletes Flash Cookies, it's not that necessary but I like to use it just to be sure.
Also, I'd like to apologize if you can't understand some pictures as they're probably going to have some Estonian in them :3
So let's get started !
1. Open www.Youtube.com and press on create account. Assuming you've already installed Cookie killer, open up the addon minibar and click on the little icon in the bottom.
Now a huge dropdown box should open with all your cookies, simply pick "Kill Cookies for youtube.com" which should be on the top if you have the youtube tab open. You should also clear them for google, after pressing create an account on youtubes official page.
After that, open up firefox addons tab located here or just press CTRL+SHIFT+A :
Then find better privacy and click on settings. A small window will pop up, now press "Remove all LSOs" which will delete all your flash cookies.
Now it's time for proxies.
I'd recommend using either private proxies ( or private shared proxies ) or not proxies at all. Public proxies tend to be massively abused and will not help you much, but if you do wan't to try them out, I'd recommend following this tutorial here : http://www.blackhatworld.com/blackha...pl-stupid.html
Or simply fetch some proxies from : http://hidemyass.com/proxy-list/
I personally don't use proxies unless I have access to some private ones.
2. Now that we have our prework done, to the actual making of the account..
Open up Pinger - it's a website where you can get a free phone number to accept text messages on.
Press sign up for free and enter random information, you don't need to confirm an email even. Just remember/write down the username and password.
After that, you'll be prompted a zip code. I usually just make up random 5 numbers and if it accepts it I'll move on.
Also, put an age above 18 as I believe that's a requirement. Gender doesn't really matter either...
Now you'll be prompted to pick a phone number
Just pick one randomly, they're all good
Now, open up youtube again and press create account on the top of the site.
Fill in the blanks, simple enough.
Use some random name, I like using famous peoples names and just add in a few numbers or change it up a little.
When you get to the mobile phone part, pick USA from the dropdown menu and open up pinger again. Click on options in pinger and copy your phone number from there. For me it's (812) 250-5782.
Just paste it in there, and finish filling in the blanks. Put the country as USA as well. You DO NOT need an US proxy.
After that just click next step twice.
Note that if you put an unique, but a name that makes sense you might not be prompted to do phone verification.
Example if you put John Smith you will almost always get it, but if you put Ricky McWhitish you're more likely to avoid it.

I've made several accounts and not one time have I been prompted to verify it.
Basically, if you do get the verification screen, take the same pinger number you used for registration and put it in the blank.
Set country as USA and pick "Text verification"
Now you'll hear a small beep on textfree and should see a phone number on the side of pingers site.
Click on that and you'll see the text google sent you, copy the PIN to youtube and you're set to go.
A few extra tips to keep your account lasting longer :
Always upload an unique avatar. I use random google images related to my niche, works like a charm.
Mess with the profile settings, hide amount of subscribers, edit the template a little bit, hide friends etc ( basically everything that shows it's a fake, new account )
Note that the last steps are only necessary if you're planning to upload some HQ content, not if you're planning to spam the shit out of the accounts with mass video blaster or rating comments.
