2) Register.
3) Fill in the subject. Anything generic will do.
4) Choose your product.
5) Enter in the description of what 'went wrong'. For an example:

Hello,
I recently got this PRODUCT from Amazon and when I went to use it, it didn't work... I contacted Amazon and they told me to contact you.
I went through all the troubleshooting steps and the PRODUCT still doesn't work at all. No response at all.
Please help
6) Don't use my example word for word. Mix it up a little bit or they'll be on your trail.
7) Now is a good time to attach proof that you purchased your item from Amazon into the attachments menu. I recommend finding and editing an Amazon receipt. You can do this via Google Chrome's inspect element, or use one of the receipt generators floating around the internet. Don't worry about the order number. In most cases it can be whatever you like, they rarely check it. (For bonus points, make it a valid order number from some random product purchase. Anything will do, really.)
8) You will get an email asking for more details and another email saying that a RMA case has been created.
9) You will eventually get an email saying that you need to ship your product back. Make sure they don't say they are giving you a box to ship it in. If they do, you can still do it but I don't think it will work well.
10) Wait a few days and email them and ask if they have received your product. They will say no. (No shit, you haven't sent anything.)
11) Wait a few days and call them and ask what's happening. You will have to give the RMA number etc.
12) They will say that they have not received it and ask for a tracking number. Say that you never got one but you will go down to the post office and try and get one.
13) Call back later giving a reference number (so that you can get connected to the correct representatives. You get this number at the end of any call) and say that they couldn't give a tracking number due to (insert reason here. The more believable the better.)
14) They will tell you that they have received the product and they have shipped a new one. (They haven't really, they just don't want to look incompetent.
15 ) Receive your product.
PLEASE NOTE:
Don't be stupid. Refer to whatever resources you can find so that your case sounds believable. A good one would be to check if your product is within warranty, if you provide a stupid fake purchase date, they will quickly dismiss you and it'll be a bitch to start the process from step 1.
NOTE 2: I used this method approximately three months ago. It's been a long time, I'm not sure if they've become aware of this.
